Health and Social Care Apprenticeship (Domiciliary) – Basildon

To provide high-quality, person-centred care and support to individuals in line with their care plans, promoting independence, dignity, choice, and wellbeing while ensuring their safety at all times. The role involves supporting individuals with daily living activities and contributing to a positive, caring environment. To report to the office team observations in the community. To review Care plans. 

Key Responsibilities:

• Deliver person-centred care tailored to each individual's needs and preferences.
• Assist with personal care, including washing, dressing, toileting, grooming, and oral hygiene.
• Support individuals with eating, drinking, meal preparation, and nutrition.
• Administer or assist with medication in accordance with company policies and training.
• Encourage and promote independence and life skills wherever possible.
• Support individuals to access the community, attend appointments, education, employment, or social activities.
• Maintain accurate, clear, and timely care records and documentation.
• Monitor and report any changes in an individual's physical or mental health.
• Safeguard vulnerable adults and children by recognising and reporting concerns promptly.
• Follow infection prevention and control procedures, health and safety legislation, and moving and handling practices.
• Communicate effectively with service users, families, colleagues, and healthcare professionals.
• Respect confidentiality and comply with data protection requirements.
• Provide emotional support, reassurance, and companionship.
• Participate in team meetings, supervision sessions, and mandatory training.
• Promote equality, diversity, dignity, and respect in all aspects of care.
